Sears Home Town Survey Notes
- The promotion is only open to legal residents of the 50 United States, Washington D.C and Puerto Rico who are 18 years of age or older and have recently visited a Sears Hometown location.
- Employees of Sears Hometown and Outlet Stores Inc and immediate family members are not eligible to win and enter the sweepstakes
- Entry into the sweepstakes is only limited to three entries per person per one monthly draw regardless of how many purchases are made at Sears
- To take the survey customers must visit a Sears Hometown store and make a purchase
- The customers will need to provide the sales check number, associate number and purchase date found on the Sears Hometown receipt
- Answer the survey questions with regards to the shopping experience and customer service provided by the Sears employee
- Enter the contact information (Name, Email address and telephone number) to gain an entry into the sweepstakes
