What is required in order to upload documents?: Completed Sworn Claim Affidavit & Proof of Loss Statement – Copy of a Government Issued ID (i.e. Stated ID, Drivers License, Passport) – If required, copy of Proof of Purchase for the claimed device (i.e. customer receipt, UPC code form a PO Box) – If required, copy of supporting documents (i.e. insurance policy, court documents, police report).

How many claims do you get with AT&T Asurion?

You can file two claims within any consecutive 12 months with a maximum device value of $2,500 per occurrence.

The documents can be digital pictures or scanned for upload.  AT&T document upload also allows the customer to fax in the documents and AT&T will upload them (this is a convenient service but the documents will take longer to upload).
