Cheddar’s Tell CV
- Take a short customer survey in regards to a visit to Cheddar’s Scratch Kitchen
- The survey will take between 5 to 10 minutes to complete
- All feedback will be held confidential and will help Cheddar’s to continually improve the customers experience
The Cheddar’s Tell CV guest survey will require the servers name and the 16 digit survey code in order to being. The servers name and the 16 digit code can be found on the Cheddar’s Scratch Kitchen customer receipt (the servers name will be found at the top of the receipt while the 16 digit code will be located towards the bottom). The customer MUST complete the Cheddar’s guest survey within 72 hours of their purchase of the 16 digit survey code will become void. Those who do complete the survey will be entered into a sweepstakes for a $300 cash prize. In order to enter the Tell Cheddar’s survey sweepstakes the customer must be 18 years of age or older and a legal resident of the United States. No purchase is required and those looking to enter without purchase can write to: Cheddars Casual Cafe Inc., “Customer Satisfaction Survey” 90 Rockwood Place, Rochester, NY 14610… please include a 3 x 5 index card with the contestants contact information (name, address, day and night telephone numbers) and a short statement about Cheddar’s Scratch Kitchen. One grand prize will be given away per month and the winner will have three days to claim the $300 cash prize. Drawings will take place on the 10th of each month and The American Sweepstakes & Promotion Co. will oversee the prize draw. Each winner will be notified by telephone and/or USPS mail, or other home delivery service of Sponsor’s choice.
