Big Y customer satisfaction survey is a survey carried out by the company to determine the satisfaction of the customers have with the services offered to them and to help improve overall sales performance. Through the survey, customers provide feedback in regards to a variety of customer relation issues to include products, employees attitudes and store cleanliness. Upon completion of the survey the customer will be entered into a monthly draw, where the lucky winner will take home a Big Y gift card worth $250 to be used on a future visit.

- Please have the sales receipt from one of the recent shopping trips to a Big Y store and the 11-digit express card number found on the receipt
- Select the Big Y store you shopped at
- Answer a few short questions (total time is about 5 minutes)
