Store Survey Rite Aid
- Take a short survey in regards to a purchase at Rite Aid
- The survey is available in English or Spanish and should take about 8 minutes to complete

The Rite Aid Store Survey requires the customer to have the 16 digit survey code along with the cashier code found on the Rite Aid customer receipt. Please note the 16 digit survey code can be found at the bottom of the Rite Aid receipt and cashier code can be found at the top of the receipt (please do not enter the “#” sign when entering the cashier code number). The Store Survey Rite Aid cashier code will be 8 digits long and will consist of all numbers. Once the survey has been completed in full the customer will have a chance to enter the Rite Aid “Voice of the Customer” Sweepstakes for a chance to win the $1,000 cash grand prize to be awarded in the form of a check. Please note the $1000 cash prize is subject to tax by Uncle Sam aka the US Government. Besides the $1000 grand prize 10 lucky winners will take home $100 cash (also to be awarded in the form of a check). Those who would like to enter the promotion without making a costly purchase at Rite Aid can write to: Rite Aid “Voice of the Customer” Sweepstakes, PO Box 16440, Rochester, NY 14616… please include proper postage along with a return address and the customers name, phone number, and email address. In order to enter the Store Survey Rite Aid sweepstakes the contestant must be 18 years of age or older and a legal resident of the United States of America.
