The Tell BI-LO survey requires the survey code from the BI-LO customer receipt and working Internet connection. The survey code will be 18 digits long and can be found in the middle of the receipt just above the bar code. Assuming the customer completes the survey in full they will receive a coupon code for $5 off their next visit at BI-LO when $40 or more is spent. Please note when completing the Tell BI-LO customer survey their research partner will collect information which may include personal information (such as the customers name or email address), transaction information (such as information from a store receipt), as well as the feedback response (the customers personal information will never be sold to a third-party or released to the employees who serviced them).
